The nurse is caring for a 6-month-old child who has elbow restraints ordered. The nurse should:
 
  a. Remove both restraints with supervision
  b. Examine both limbs one at a time with the restraints off
  c. Keep the restraints on until the physician orders them to be removed
  d. Generously pad the restraints, but not remove them

Question 2

The nurse monitoring the TPN infusion for a small child assesses that the infusion is behind. The nurse should:
 
  a. Speed up the infusion to catch up
  b. Notify the charge nurse
  c. Stop the infusion, and notify the charge physician

TPN must be monitored carefully. Speeding up an infusion can cause hyperglycemia. The infusion should not be stopped. The charge nurse should be notified.
