Answer to Question 1

Answer:3
Rationale: Although all of these factors are preventable causes of mortality in the United States, tobacco use accounts for 430,000 deaths annually and is the most preventable cause of adult death.

Answer to Question 2

Answer:3
Rationale:Elevated lead levels are neurotoxic to young children and if untreated can cause irreparable neurological damage. Visual acuity may be difficult to accurately assess at this age secondary to the child's compliance and ability to understand the directions for the screening test. While teaching helmet use at an early age is important,it is unlikely that this child is riding a bicycle yet,and although early exposure to fluorine is important for good dental health, lack of fluorinated drinking water will not be as harmful to the child as toxic lead levels.
