Answer to Question 1

D
The good eye is patched to force the child to use the bad eye, thus strengthening the muscles. The patch should always be applied directly to the child's face, not to eyeglasses, and should be left in place even when the child is sleeping. Covering the bad eye will not contribute to strengthening it. The good eye should be patched.

When taking children's vital signs, the nurse observes the respiratory rate first. Temperature should be measured after other vital signs because it can be upsetting for children. Heart rate is not the first vital sign measured in children. Blood pressure is taken after other vital signs because it can be upsetting for children.
