The nurse is caring for a 16-year-old adolescent who just delivered a baby.
 
  The new mother, while filling in the birth certificate, tells the nurse that she plans to leave the father's name section blank because he doesn't want anything to do with the baby. The nurse's best response would be: A) You have the right to leave that section blank, but it will make it more difficult if you should ever decide to attempt to obtain financial support from him in the future. B) I don't blame you. I would do the same thing in your situation. C) You must fill in the name, or the form will not be accepted. D) You really should fill in the name of the father.
