Answer to Question 1

B
The most important finding is that the palate is closed and there are no clefts. Assessing sucking capability is not the most important finding of this procedure. Assessing for candida albicans infection is done by visual inspection.

Answer to Question 2

C
You recognize that further teaching is needed if the nurse uses her bare hands to press on the woman's breast. The new graduate nurse should have put on gloves before beginning the assessment of the woman's nipple discharge.
