The client should be reassured that feeling flushed and warm are common in clients receiving IV magnesium sulfate. Performing a vaginal exam to determine progress and stopping the drug and calling the physician are not appropriate nursing actions. Turning the client on her left side and increasing IV fluid is not an appropriate action because feeling flushed and warm are common in clients receiving IV magnesium sulfate.

Thick and greenish-black stools are characteristic of the meconium stool, which is the neonate's first stool. Later stools may become loose and green or yellow and seedy.
