Charting the findings is an important intervention, but it is not the priority action. Checking for clonus is not an appropriate action at this time. Decreasing the magnesium sulfate is not a nursing intervention without health care provider prescription.

The nurse should report the fact that the neonate has not voided. The neonate should void by 12 hours after birth. While it is appropriate to pass on information regarding the milky discharge from nipples, polydactyly on each hand, and undescended testes; this information is not the most essential to note.
