Assessment is the first step in the nursing process and is used to collect and analyze the data, so that a plan can be developed appropriate for the client. Identifying nutritional risk factors is not possible until assessment is performed. Basic prenatal nutrition teaching is the responsibility of the nurse, not the dietitian. Teaching should not be performed until after the assessment is made.
