During the first (initial) feeding, a client asks how long her baby should suckle on each breast per feeding. Which is the nurse's best response?
 
  a. Five minutes per side for each feeding will empty each breast and prevent nipple tenderness.
  b. Keep waking the baby up until he nurses at least 15 minutes at each breast every 3 hours.
  c. Nurse only on one breast at each feeding today for 5 minutes per feeding and start alternating breasts with the first feeding tomorrow.
  d. The time will depend on your baby's re-sponse. Increasing to about 15 minutes per side will be helpful to establish milk supply.

Nursing the infant to a goal of 15 minutes per side will provide adequate milk supply. Five minutes per side is not adequate time for the breast to be emptied. Unless there is a pathophysiological problem with the infant, waking the child every 3 hours shouldn't be necessary. Normally, the infant feeds on demand every 2 to 3 hours. Nursing only on one breast at each feeding today for 5 minutes will encourage engorgement.
