Answer to Question 1

B
Decreased fetal movement should signal the client to notify the health care provider immediately. Constipation is one of the discomforts of pregnancy due to hormonal and pressure changes, with fluid and exercise needs continuing throughout pregnancy. Early evening fatigue is a common discomfort of pregnancy. Loss of appetite may be a result of nausea and vomiting.

Answer to Question 2

A
Brushing the infant's lips and mouth area with the nipple will stimulate the rooting reflex. Pulling down on the infant's chin will stimulate continued mouth closure by the infant. Stroking the infant's cheek gently may also stimulate the infant to root, but not as effectively as brushing the baby's lips and mouth areas with the nipple. Tickling the infant's feet or toes will stimulate the infant to wake up, but will not affect the opening of the mouth.
