A five-year-old child is on chemotherapy for rhabdomyosarcoma. Despite antiemetics, the child complains of nausea. The mother wants the child to eat and is pushing the child to eat the food.

The nurse would talk with the mother and suggest that she not push the food on the nauseated child because: 1. The child does not need to eat as he is on intravenous fluids.
  2. Forcing the child to eat may lead to a food aversion for the child.
  3. Vomiting can lead to damage to the stomach.
  4. Pushing the child to eat leads to a psychological conflict that may turn the child away from the parent.

Question 2

The nurse is providing anticipatory guidance instructions to the parents of a newborn. Which of these instructions should the nurse give as disease/injury prevention strategies? Standard Text: Select all that apply.

  1. SIDS risk reduction
  2. Fall prevention
  3. Formula safety
  4. Immunization schedule
  5. Risk of poisoning

Answer to Question 1

2
Rationale 1: Intravenous fluids do not replace normal food intake.
Rationale 2: If the child is forced to eat and then vomits, the child can develop a food aversion in which the child associates that food with vomiting.
Rationale 3: Vomiting is unpleasant but does not usually lead to stomach damage.
Rationale 4: This is not a correct statement. Parents and children often disagree, but the child will still relate to the parent.

Answer to Question 2

1,2,3,4
Rationale 1: SIDS is a major concern of infancy, and information on SIDS prevention, including back sleeping and the use of fans for air movement, should be provided.
Rationale 2: Anticipatory guidance should be given about fall prevention. Newborns are incapable of protecting themselves from falls and must always be placed in a secure environment, including the use of bed rails and safety straps when appropriate.
Rationale 3: Parents must be instructed on safe handling of formula including preparation and storage guidelines.
Rationale 4: It is important for parents to understand the recommended vaccination schedules and to comply with these standards to protect their child from childhood illnesses.
Rationale 5: It is too early for a discussion on the risk of poisoning. Until the child is mobile, poisoning is not a high priority for client teaching.
Global Rationale:

Every flu season is different, and even healthy people can get extremely sick from the flu, as well as spread it to others. The flu season can begin as early as October and last as late as May. Every person over six months of age should get an annual flu vaccine. The vaccine cannot cause you to get influenza, but in some seasons, may not be completely able to prevent you from acquiring influenza due to changes in causative viruses. The viruses in the flu shot are killed—there is no way they can give you the flu. Minor side effects include soreness, redness, or swelling where the shot was given. It is possible to develop a slight fever, and body aches, but these are simply signs that the body is responding to the vaccine and making itself ready to fight off the influenza virus should you come in contact with it.
