The nurse is speaking with a group of adolescents about what can happen when having unprotected sex. The nurse understands that to communicate effectively with teens, she must:

  1. Allow for discussion, and bring peers who have had experience related to the topic being discussed.
  2. Lecture on the topic for the allotted time without any discussion.
  3. Offer personal opinions on the topic, and cite examples of what can happen if they don't listen.
  4. Discuss sex education related to religious belief.

Question 2

A child who is medically fragile is being discharged home after a long hospital stay. The child will require someone to provide medical treatments several times a day. In planning discharge, the nurse will:

  1. Train all members of the family to take part in the child's care.
  2. Plan for a home health nurse to visit the child four times a day to provide the medical treatment.
  3. Determine with the family which family members may be trained to provide care for the child.
  4. Evaluate the size of the child's room for therapy.

Answer to Question 1

1
Rationale:
1. Whatever the setting, the nurse partners with the adolescent, the parents, and other persons such as teachers or school counselors to plan appropriate goals and related intervention. Apply communication skills effective with teens, such as listening to concerns, allowing for discussion, and bringing peers who have had experiences related to the topic being discussed.
2. Lecturing without discussion will not draw in the adolescent to the content.
3. Personal opinions will not carry much weight with a group of adolescents.
4. Discussing sex education from a religious viewpoint is not appropriate.

Answer to Question 2

3
Rationale 1: Training all members to participate in the care of the child might be inappropriate based each member's age, abilities, and developmental stage.
Rationale 2: It is impractical to expect a home health nurse to visit more than once a day. The nurse should determine if family members can be trained to provide the needed care.
Rationale 3: Not all members of the family will be able or willing to provide care. One parent, both parents, or members of the extended family may be the best option for home care.
Rationale 4: The size of the child's room has no bearing on the child's care as given by the family members.
