Answer to Question 1

ANS: C, D
The FHR should be monitored for at least 30 minutes for changes and the uterus should be assessed for excessive contractions. To reduce leakage, the woman should lie flat for 15 to 20 minutes after the gel form of prostaglandin is inserted. The client should not be assisted to the bathroom immediately or placed in a high Fowler position (head of bed up 90 degrees).

Answer to Question 2

ANS:
8
The Bishop system is used to estimate how easily a woman's labor can be induced. Higher scores are associated with a greater likelihood of successful induction because her cervix has undergone prelabor changes, often called ripening. A cervix dilated to 1 cm = 1 point; a cervix effaced 50 = 1 point; fetal station of 0 = 2 points; soft cervix = 2 points; and anterior positioned cervix = 2 points; total points = 8.
