   1. Slanting and circular, twisting around the bone shaft

The correct calculation is:

66 lb/2.2 kg = 30 kg

Dose of Plaquenil is 5 mg/kg/day divided bid

5 mg  30 = 150 mg

150 mg/2 = 75 mg
