For individuals who are severely injured or have a serious chronic illness, hospitals are often the entry point into the rehabilitation system.
Home services: This program includes the following services.
Meals on Wheels: This program provides hot and cold meals to housebound recipients who are incapable of obtaining or preparing their own meals but who can feed themselves.
Home health services: These programs provide visiting nurse services, drugs, physical therapy, laboratory services, and sickroom equipment.
