No. The nurse cannot use an unlabeled reconstituted medication because it may have expired and be ineffective, decomposed, or contaminated.

Patient name if needed, Date of reconstitution, Reconstituted strength, Nurse preparer, and Re-constitution expiration date and hour if agency requires.
