Order: Heparin sodium 1,000 units per hour, increase by 200 units if results of INR remain subtherapeutic to a maximum of 2,000 units per hour. The solution is mixed 20,000 units in 1000 mL.
 
  Calculate the initial flow rate in mL/hr and construct a titration table for this order.

Question 2

The order is to infuse heparin at 315 units/hr using a solution strength of 14,000 units in 250 mL D5NS. Calculate the flow rate in mL/hr.
