The nurse must administer Narcan (naloxone hydrochloride) to a neonate who is experiencing respiratory depression. The prescription is 0.01 mg/kg subcut stat for this neonate who weighs 3,300 g. The strength available is 0.4 mg/mL.
 
  How many milliliters will you administer? Round the answer to the nearest hundredth.

Question 2

The pediatric client is prescribed Dycill (dicloxacillin sodium) 50 mg po q6h. The recommended dosage for a 35-pound child is 12.525 mg/kg/d in 4 equally divided doses. The concentration of the medication is 50 mg/5 mL.
 
  If the prescribed dose is safe, how many mL will the nurse administer? Round answer to the nearest whole number.

0.08 mL

Global Rationale: mL = 1 mL/0.4 mg x 0.01 mg/1 kg x 1 kg/1000 g x 3300 g/1 dose. Multiply the numerators and the divide by the denominators. The nurse will administer 0.08 mL of medication to the neonate.

Global Rationale: First, determine the safe dose range for this child. mg/dose = 12.5 mg/1 kg x 1 kg/2.2 lbs x 35 lbs/day x 1 day/4 doses = 49.7 mg. mg/dose = 25 mg/1 kg x 1 kg/2.2 lbs x 35 lbs/day x 1 day/4 doses = 99.4 mg. The prescribed dose of 50 mg is safe; therefore mL/dose = 5 mL/50 mg x 50 mg = 5 mL.
