The prescriber orders a medication q8h. The nurse enters the following times on the medication administration record (MAR): 12 AM- 6 AM- 12 PM- 6 PM.

ANS:
The wrong time intervals, the time intervals are 6 hours apart instead of 8 hours. The times if not corrected will cause the client to receive one more dose of the medication than was intended.
