Directions: For the following medication order indicate what component of the order is missing.
 
  Trental 400 mg po.

Question 2

The prescriber orders a medication q8h. The nurse enters the following times on the medication administration record (MAR): 12 AM- 6 AM- 12 PM- 6 PM.
 
  What error has occurred?

ANS: Frequency

Answer to Question 2

ANS:
The wrong time intervals, the time intervals are 6 hours apart instead of 8 hours. The times if not corrected will cause the client to receive one more dose of the medication than was intended.
