Order: Potassium chloride (KCl) 0.9 mEq to be added to each IV q8h for an infant weighing 6 lb. The safe dosage range is 1 to 4 mEq/kg/24 hr, not to exceed 40 mEq/day.
 
  The safe pediatric rate is 0.5 to 1 mEq/kg/hr, not to exceed 10 mEq/hr. KCl 0.45 mL was withdrawn from a vial labeled 2 mEq/mL and further diluted to a total volume of 10 mL NS in a Buretrol. The IV infusion pump is infusing at 22 mL/hr. What is your course of action? ___________________ ___________
