Paul is an individual who tends to dominate group therapy sessions. Baseline data reveal he has a high frequency of episodes of interrupting others and changing topics to focus on his issues. His therapist discusses this with Paul outside the group session and learns Paul is concerned he will not get to discuss his own concerns if he does not speak up and have others listen to him. The therapist tells Paul she will try two different approaches with him during the morning and afternoon sessions. In one session, she will allow Paul to discuss his own concerns first with her for 30 minutes. He will then listen attentively to others and contribute more appropriately. In the second session, she will allow Paul to discuss his concerns after the session with her for 30 minutes and he should contribute appropriately in that session. However, regardless of his behavior in the group sessions, the therapist is obligated to provide Paul with his personal sessions.

What is a primary disadvantage to allowing Paul to have his personal sessions regardless of his behavior in group therapy?

Answer to Question 1

Assuming Paul likes the personal sessions as a rule, the therapist, in a sense, is delivering the sessions noncontingently. In other words, Paul does not have to demonstrate appropriate contributions in the group session prior to having his personal session. Paul could have his personal session and still dominate the group session. However, given Paul is need of these personal sessions, it would also be inappropriate to withhold the sessions. Therefore, the therapist is trying to determine if the timing of the personal sessions might influence Paul's behavior in the group session by ensuring him he will have opportunity to discuss his concerns aloud with another person.

Answer to Question 2

Clearly the order of the interventions, day of the week, and time of day are critical (e.g., morning v. afternoon session). Additionally, she would need to ensure her personal therapy sessions were as similar to one another as reasonable regardless of which time was in effect. Third, she might need to also counterbalance group session topics if she was suggesting specific areas for the group to discuss.
