Paper and pencil acuity systems done by nurses had a propensity for acuity creep, in which nurses misrepresented patient needs by padding responses to increase the staffing. Although it did take a great deal of time to collect and score the data, this was not the main problem. Acuity systems never predicted the length of stay.
