Answer to Question 1

B
Two primary criteria make for effective decisions. First, the decision must be of a high quality; that is, it achieves the predefined goals, objectives, and outcomes. Second, those who are respon-sible for its implementation must accept the decision. Higher-quality decisions are more likely to result if groups are involved in the decision-making and problem-solving process. Taking own-ership of the process and outcome provides a smoother transition in changes.

A patient safety practice is a type of process or structure whose application reduces the proba-bility of adverse events resulting from exposure to the health care system across a range of con-ditions or procedures (Shojania et al., 2001, p. 29).

Chronic necrotizing aspergillosis has a slowly progressive process that, unlike invasive aspergillosis, does not spread to other organ systems or the blood vessels. It most often affects middle-aged and elderly individuals, spreading to surrounding tissue in the lungs. The disease often does not respond to conventionally successful treatments, and requires individualized therapies in order to keep it from becoming life-threatening.
