Answer to Question 1

D
Simulation has been recommended by the Institute of Medicine's report To Err Is Human: Building a Safer Health Care System (Kohn, Corrigan & Donaldson, 2000) and Durham and Alden (2008) as a teaching strategy to promote patient safety, provide opportunities to intervene in clinical situations without risk to a patient, and a way to experience specified scenarios in a supervised safe setting. The HyGreen system (not simulation) is used to promote hand hygiene compliance and utilizes sensors to detect whether a person has used hand sanitizer in a specific setting.

Answer to Question 2

B
The Institute of Medicine (IOM) report, To Err Is Human: Building a Safer Health Care System (Kohn, Corrigan & Donaldson, 2000) recommended the use of simulation training as one strategy that can be used to prevent errors in the clinical setting. Simulation has been recognized to promote teamwork, effective communication, and ways to introduce new equipment or protocols in a safe nonthreatening environment.
