Unlimited public access, mixed with other risk factor, makes the hospital an unsafe workplace. The other option are not risk factors for violence.

Answer to Question 2

ANS: A, C, F
Options A, C, and F are correct because inaccuracies of shift reports can be eliminated by using walking rounds. Options B, D, and E would cause interruptions in the report and lead to walking rounds.
