Answer to Question 1

Correct Answer: 3

Data are discrete entities that are described objectively without interpretation. Data are transformed into a knowledge base for the nurse to utilize to assess and implement care. After care interventions, nurses collect additional data to evaluate the care given and patient outcomes. Synthesized information is also referred to as knowledge. Related concepts are processes that have key elements linking them together. Information is data that has been interpreted, organized, or structured.

Answer to Question 2

Correct Answer: 1

The components of the communication process are sender, message, channel, receiver, and response or feedback. The manager in this case responded inaccurately, but did respond by scheduling the vacation day. There is no requirement that the response be what was expected by the sender. The response does not have to occur in the same channel or format in which it was sent.
