Research has demonstrated that home-based approaches to care delivery produce better outcomes for certain populations. Population-focused home care is directed toward:
 
  1. The needs of specific groups of people with high-risk health needs
  2. Acutely ill adults in the home
  3. Clients who are on home ventilators
  4. People in rural areas with no access to care
