Answer to Question 1

Ans: C
To construct a monitoring chart for a program plan, information is needed on the inputs (resources necessary to carry out the program, such as personnel, equipment, and finances), the process (the program activities, their sequencing, and timing), and outputs (the expected results of the program, including immediate and long-term health effects).

Answer to Question 2

Ans: B
Surveys that are used for program evaluation must be concerned with the reliability and validity of the information collected. Reliability deals with the repeatability, or reproducibility, of the data (i.e., if the same questions were asked of the same people 1 week later, would the same responses be recorded?). Validity is the correctness of the information. Variability refers to the difference between the highest and the lowest piece of information gathered in a study. It may also be known as the standard deviation. Consistency is the ability to repeat exactly the same results. This is different from reliability, in that reliability connotes some degree of variation in the results while still maintaining validity of the results.
