Hospitalized older adults require early and competent discharge planning to prevent complications, reduce the risk of rehospitalization, and minimize stress to themselves and their caregivers. It is not a goal of early and competent discharge planning to cause patients to leave the hospital sicker and in a more debilitated state that is a consequence of payers wishing to avoid paying for extended hospital stays.
