Answer to Question 1

A
The nurse is alert for rough, dry, flaky skin when an older adult complains of pruritus to be able to prevent linear excoriation leading to skin breaks, excoriation, inflammation, and infection. A brown macule is a freckle or a liver spot, an indication of sun exposure. Brownish skin is a clinical indicator of venous insufficiency. Regional edema is a sign of fluid overload and venous in-sufficiency; localized edema is a sign of infection.
