Answer to Question 1

C
Response C is the best answer; infection and dehydration are reversible causes of confusion, and Ms. Jones' mental status is likely to improve if these problems are reversed. Responses A, B, and D do not demonstrate knowledge of the impact of dehydration and infection on Ms. Jones' mental status. IV therapy can be frightening, but it would not be the only cause of confusion.

Answer to Question 2

D
Gathering resident care data and documenting your findings before confronting Suzie about an unacceptable behavior would strengthen the rationale for placing Suzie on probation (response A) or initiating termination procedures (response B). Writing a detailed narrative of the complaint (response C) would be a good idea, but it is not as important as having first-hand information regarding the problem.
