Answer to Question 1

D
The CNA often gives the personal care to the resident under the supervision of the LPN. The LPN manages the care given by the CNA and passes medications, does dressings, and provides other care within the LPN domain of practice. The RN is identified as head of the nursing team.

Answer to Question 2

Dementia has a gradual onset that may be detected only by an annual cognitive examination. Depression (response A) and elevated blood pressure (response B) would not cause a change in mental status. Decreased cognition is not a normal aging change (response C).
