Answer to Question 1

A
Omnibus Reconciliation Act (OBRA) guidelines mandate 30-day review after recent admission and quarterly (90-day) interdisciplinary team review. These time frames provide measurable time markers for goals. Responses B, C, and D are choices for measurable time frames but usually are not used because of the lack of framework for reviews at these intervals.

Answer to Question 2

C
Because of Mrs. Jacobs' increasing confusion and incontinence, a skilled nursing facility would be the most appropriate placement. Hospitalization (response D) is inappropriate because Mrs. Jacobs is not experiencing a life-threatening process that needs intensive medical evaluation and treatment. A retirement center (response A) and an intermediate care facility (response B) would not provide enough supervision for Mrs. Jacobs.
