Answer to Question 1

A
The change in accommodation to light and dark takes longer and increases the incidence of falls for older adults. Responses B, C, and D do not place an older adult at risk for falling.

Answer to Question 2

C
Senile purpura is a vivid purple patch with good borders. It is a normal skin change for elderly adults. It will fade. Responses B and D are unnecessary at this time because this is not related to falls or foods. You should call for a physician's appointment if the client is still anxious about the new lesion.
