Because a BVM is essentially a volume-controlled ventilator, an increased resistance or decreased compliance should not effect tidal volume, unless more leaks develop or the bag distends. However, to overcome these changes in lung characteristics, the therapist needs to use two hands to squeeze the BVM, assuming that another rescuer can hold the mask in place to create a seal during ventilation.

The rescuer kneels and places the head of the victim between his knees. One hand of the rescuer holds the mask and provides the head-tilt/chin-lift by pulling the patient's mandible upward. The rescuer's remaining hand squeezes the bag against his own thigh in an attempt to maximize bag deflation.
