CPAP can cause pulmonary overdistention, which can increase WOB and cause CO2 retention. Volutrauma and VILI are associated with invasive mechanical ventilation. Oxygen toxicity is related to the PaO2 that results from FIO2 levels higher than 0.6.

A newborn with a patent ductus arteriosus has increased pulmonary blood flow and reduced lung compliance and FRC and therefore would benefit from the positive intrathoracic pressure pro-duced by CPAP. The use of CPAP can be dangerous in newborns with choanal atresia, a trache-oesophageal fistula, or a cleft palate.
