The first mechanism is the ability of NIV to rest the respiratory muscles. Second, NIV lowers the PaCO2, and the decrease is believed to reset the central ventilatory controller and establish a new baseline PaCO2. The third mechanism is the improvement in lung compliance, lung volume, and dead space that result from NIV.

The CPAP group had a lower rate of intubation, pneumonia, overall infection, and sepsis.
