The external diameter of a hi-lo jet tube is approximately equal to the external diameter of a standard endotracheal tube that is a half-size larger. Therefore, hi-lo tube size = standard endo-tracheal tube size  0.5 mm; 4.0 mm is standard endotracheal tube size, so 4.0 mm  0.5 = 3.5-mm hi-lo tube size.
