Answer to Question 1

Notify the nurse immediately anytime there is a change in urine output and when a patient does not make urine for more than 4 to 6 hours. A patient should make at least 30 mL of urine an hour. If he does not, it should be reported to the nurse. This could be a sign of kidney failure and needs to be properly addressed.

Answer to Question 2

Persons with intellectual disabilities must be protected from abuse and may be fearful or unable to communicate when it occurs. Signs of abuse include increased anxiety, changes in mood or behavior, frequent injuries, and injuries in different stages of healing. Notify the nurse of your observations and concerns immediately.
