You are assisting with a 24-hour urine collection on a patient assigned to your care. The patient is in the dining room and chooses not to return to his room to collect the specimen; instead he voids in the nearest bathroom. Is it okay to just collect one extra sample and continue the collection?

A patient you are caring for is receiving a bolus tube feeding through a nasogastric (NG) tube. You observe the patient is comfortable and resting in the supine position. Should you take any action?

Answer to Question 1

The test must be re-started as a void collection was missed. The results will no longer be valid. Notify the nurse so additional teaching with the patient can be done and he fully understands. Re-start the process following the 24-hour urine specimen procedure.

Answer to Question 2

Patients receiving tube feeding are at high risk for aspiration, and the head of the bed should be elevated during and after the feeding. They should be in a Fowler's or semi-Fowler's position. The bed should be elevated and the nurse should be notified.
