Generally, a physician's order is required to apply restraints. If restraints are necessary, as in the example stated, they should be used only after alternatives have failed to help. Restraints should never be used for the nurse's convenience or for understaffing situations. Side rails are another form of restraint. Both side rails should not be up on all beds unless proper justification exists.
