Answer to Question 1

C
Feedback:
A nurse can place all the medications in appropriate doses in organizers for a week or a month. This helps the client to remember if the medications have been taken or not and also to take medications in prescribed doses. Using organizers encourages self-care by the clients. The nurse or a volunteer need not provide medications to the client every day. A list of prescribed medications is not of much help to the forgetful client. The list must also mention the dose and time when the medications should be taken.

Answer to Question 2

C
Feedback:
Some tumors may be removed by en bloc resection, which involves removal of the tumor and surrounding tissues and lymph nodes. Larger tumors may be treated by exenteration (removal of the tumor, the organ involved, and surrounding tissues). An excisional biopsy will remove the tumor as well as some of the margin of the tumor. To prevent growth and spread, malignant tissues may be frozen (cryosurgery), burned (electrocauterizatio n), or destroyed by high-frequency current (fulguration).
