The nurse should carefully observe and monitor the client with a disorder of the blood or lymph, and examine the client's skin for petechiae, bruises, or other evidence of abnormal bleeding. The nurse should also obtain a thorough nursing history that includes the client's nutritional status, dyspnea, elimination difficulties, difficulty in walking or moving, and pain. Signs and symptoms in these areas may indicate a blood or lymph disorder.
