Answer to Question 1

D
Feedback:
Determining the client's understanding of acne and its treatments are short- term goals that provide a baseline from which to develop a teaching plan and appropriate strategies. Voicing a decrease in lesions is a long-term goal following treatment.

Answer to Question 2

B
Feedback:
Lochia rubra is seen for the first 2 days. It is mostly red and bloody. It should smell like blood (slightly metallic); a foul odor indicates infection. Lochia serosa starts after the bleeding diminishes. The color of the lochia changes to pink or brown-tinged for approximately the next 7 days. Lochia serosa has a slightly earthy odor. Lochia alba, which is yellow or white, starts on about day 10 . At this point, the lochia has decreased greatly in amount. Lochia alba also has an earthy smell.
