Answer to Question 1

D
Feedback:
The nurse should determine whether the client is allergic to shellfish or iodine because the dye used in IVP is iodine based. The client should be kept NPO for 8 to 10 hours before the test, not just 4 hours. The client should take a laxative the night before the procedure to eliminate any feces or gas that could obstruct the view of urinary structures. The client should not swallow any water before the test. This could lead to inaccurate test results.

Answer to Question 2

A
Feedback:
Primary (family) caregivers are vital to the client's care. They provide a constant liaison between the client and the hospice team and may suggest approaches to care that meet with everyone's approval. Caregivers often can identify changes in the client's condition that might not be noticeable to others.
