Answer to Question 1

C
Feedback:
When taking a child's temperature, the nurse should consider his or her ability to cooperate, because if the child is uncooperative, the thermometer could break, or the reading could be inaccurate. The nurse should make sure side rails on beds and cribs are up at all times. Beds should always be in the lowest position unless performing specific procedures. The nurse should use disposable diapers unless the child is sensitive to them and the child should use only toys that can be disinfected. Families should be reminded that toys that cannot be disinfected may need to be thrown away.

Answer to Question 2

D
Feedback:
A client who has given birth many times (specifically, at least five times), is called a grand multipara. If it is her first pregnancy, she is a primigravida. If she has delivered a child for the first time she is a primipara. A client who has never given birth is a nullipara.

Dogs have been used in studies to detect various cancers in human subjects. They have been trained to sniff breath samples from humans that were collected by having them breathe into special tubes. These people included 55 lung cancer patients, 31 breast cancer patients, and 83 cancer-free patients. The dogs detected 54 of the 55 lung cancer patients as having cancer, detected 28 of the 31 breast cancer patients, and gave only three false-positive results (detecting cancer in people who didn't have it).
