Answer to Question 1

B

Answer to Question 2

A
Feedback:
When a nurse is providing home care for an ill and frail client, the nurse must suggest that the family members use an alarm system for the client to summon help. This ensures that the client gets immediate assistance if needed. A nurse should provide the client with footwear with nonskid soles made of leather or rubber rather than providing socks; socks can cause sliding. The nurse should instruct the family members not to use any rugs or cloth to cover the floor. This prevents the client from slipping or tripping over such floor covers. The nurse should do a fall risk assessment for the client's entire home rather than only the client's bedroom.

It is tightly attached to the skin below it, so it compensates for the increased area by wrinkling. This happens to the hands and feet because they have the thickest layer of dead keratin cells.
