Answer to Question 1

B
Feedback:
The primary assessment is the assessment performed as soon as rescuers arrive at an emergency scene. During this assessment, life-threatening problems or injuries are identified and handled. If there are no life-threatening problems to correct, the primary assessment usually can be completed within 60 seconds. The secondary assessment involves taking and recording the victim's vital signs and continues with a head-to-toe assessment. This secondary assessment should take from 1 to 2 minutes, unless injuries requiring immediate intervention are identified. If the person has life-threatening problems, the secondary assessment may be delayed until the person is being transported. Whenever assessing a person in an emergency, use the acronym ABCDE to help you remember the order for assessment: A = Airway and cervical spine; B = Breathing; C = Circulation and bleeding; D = Disability; E = Expose and examine. There is no tertiary assessment.

Answer to Question 2

C
Feedback:
The nurse should take measures to keep the stroller tank upright at all times because tipping of the tank will vent the oxygen contents quickly. The stroller can be refilled from a larger stationary reservoir unit, not from small oxygen cylinders. The nurse need not reduce the pressure in oxygen strollers, because they are not under extremely high pressure. Oxygen concentrators, not oxygen strollers, need electricity to operate.
