Answer to Question 1

C
Feedback:
In some cases, lights will be flashed, the client will be asked to watch a repeating pattern on a TV monitor, or a small electrical stimulus will be given. The brain's responses to these stimuli are recorded. These responses are called evoked responses or evoked potentials. The nurse should explain to the client that no electric shock will be given and that the EEG procedure cannot determine thoughts or mental ability. The nurse should also tell the person that he or she will be asked to open and close the eyes and sometimes to perform other movements on command. In preparation for an EEG, the client may be asked not to sleep the night before the test (for a sleeping EEG), or he or she may receive sedation (for a resting EEG).

Answer to Question 2

D
Feedback:
The nurse should be aware that an anaphylactic reaction manifests as difficulty in breathing, syncopal episodes, rapid pulse, and palpitations. Anaphylactic reaction is a medical emergency that the nurse must recognize and treat promptly. Medication toxicity is an undesired, harmful effect that results from an increased blood level of the medication beyond its therapeutic level. If the client's response is opposite to that which is desired, it is referred to as a paradoxical effect. Therapeutic effect is the medication's desired effect and not its complications.
