Answer to Question 1

C

Answer to Question 2

C
Feedback:
The nurse should instruct the client to maintain a regular time for fecal elimination to relieve the condition. Manual disimpaction is not possible for a client with quadriplegia. The nurse should instruct the client to consume a well-balanced diet instead of a low-protein, low-calorie diet. The nurse should also instruct the client to take adequate fluid instead of restricting the fluid.
